**Important fact for families with multiple children at Clark: Each form below can be filled out for multiple students at one time! At the bottom of the first page of each form, there is a button, “Add Another Student,” which will add another student to the form. So for example, if you have two children that you need to complete the Student Pledge for Technology Use form for, you can add the first student in the “Student #1” field, click on “Add Another Student,” then add the second student in the new “Student #2” field. Once you have all your students added, you can click “Next” and follow the onscreen directions to complete the form
